If you’d like to meet us first, we offer a FREE “Meet-and-Greet” appointment so you can tour our office, meet Dr. Akbar, and ask any questions before committing.
Finding the right pediatrician in Suwanee, Duluth, Johns Creek, Buford, Sugar Hill, or Cumming can feel overwhelming — we want to make that decision easy and comfortable for your family.
All we need to get started:
- Immunization records and any other significant medical records
- A copy of your insurance card
Give us a call and our friendly staff will guide you through transferring your child’s previous medical records to our office and registering your child for their first visit. We accept most major insurance plans and also offer self-pay rates for families without insurance. New patient appointments are available — don’t wait to get your child the quality pediatric care they deserve.
Transferring Your Child to a New Pediatrician?
We are sad to see you go! We understand that families relocate, children age out of pediatric care (21+ years), insurance plans change, or sometimes it’s simply not the right fit — and that’s okay. Our goal has always been to provide the very best pediatric care for every child, and we genuinely welcome any feedback you’d like to share with us.
To transfer your child’s medical records:
- Please complete and sign a Medical Release Form
- After we receive the completed form, please allow 5–10 business days for records to be processed and sent to your new provider
We will ensure your child’s immunization history, visit notes, specialist referrals, and medical records are transferred completely and securely to their new pediatrician.
